Project Summary:
Investigators with experience in and commitment to palliative care are proposing multimodality interventions to integrate palliative care into symptom management and provider/family communication on four tertiary care ICUs. A pre-post intervention design will evaluate use of a computer-based system to identify a subset of patients with high-level needs for palliative care, and initiate a computer-based, symptom-focused palliative care needs-prompting system directed to nurses and physicians practicing on the four units. A palliative care physician-nurse team will introduce on-unit palliative care. Focus will include symptom control, provider/family communications, and preparation of family members for advance care planning conferences.
